Following the successful evaluation of the Acute Home Visiting Service in Newark, we are excited to announce that we have been asked to expand the service to cover the practices in Sherwood until end of Sept 2018. The small team of Advanced Nurse Practitioners and Emergency Care Practitioners support GPs by providing same-day home visits when patients are unable to get to the surgery. We use our clinical skills to assess people who are acutely unwell, providing medications and accessing other services to ensure they get the right care to enable them to remain at home during their illness whenever possible. With a large, rural geographical patch to cover we are hoping for a mild winter with no snow!
