Starting on the 1st April 2019, the PICS operated Hospital to Home Respiratory service will provide a more joined up service for respiratory patients who are in hospital. The service will work with hospital clinicians at both the City Hospital and QMC to identify patients who can leave hospital and ensure the required ongoing care is provided in the local community health services. This will provide more integrated care for patients and will reduce avoidable re-admissions into hospital and support a reduction in length of stay in hospital.
Greater Nottingham Commissioning Partnership believes that this service will help tackle the current pressure on respiratory services in local hospitals and also support future winter plans.
PICS will employ three respiratory nurses for this pilot service.
The pilot will run for approximately 20 months and within this period an evaluation will take place to assess how effective the service at improving healthcare for respiratory patients. If the pilot proves successful a full procurement exercise will be undertaken by Greater Nottingham Commissioning Partnership
