Our Acute Home Visiting Service was co-designed between NHS commissioners, local GPs and PICS and has won a national award for value and innovation from the prestigious Health Services Journal Awards. In three years of running, it’s estimated to have saved patients from having to go to A&E 7,000 times, resulting in £8m savings for local hospitals. From 24,000 visits from 14 highly skilled clinicians, it’s also estimated to have saved 15,000 hours for GPs. As Dr Thilan Bartholomeuz, GP and Clinical Lead for Mid-Notts Integrated Care Partnership explains:
The AVHS service covers large areas, so when clinicians are deployed to one patient, they can see other patients nearby with much more efficiency. That means you don’t have four different GPs from neighbouring practices visiting the same nursing home at the same time. This is not only efficient use of resources but enables GPs to see more patients in their surgeries.
