PICS and the Primary Care Networks have been recognised as ‘key partners’ in the award-winning Community Care Transformation Programme. It’s the winner of Best Initiative Supporting Integrated Care Systems category in the prestigious Integrated Health Awards 2023, announced in May 2023.

Our partnership work was also honoured by being shortlisted in categories celebrating the most promising pilots in England and the best transformation programmes of 2023.

The Community Care Transformation Programme enables local partnerships to work together to plan and deliver sustainable community care services during a 100 day cycle. Each project is designed by local professionals to respond to the specific needs and circumstances of their local residents. All the projects aims to improve independence by supporting physical, mental and social needs.

Teams working together were formed in Nottingham West, Rosewood and Mansfield North, Newark and Ashfield Primary Care Networks. The partnerships include volunteers, residents, council teams and staff working for NHS services. Read more here.
