Hospital to Home Respiratory Service
Provided for people registered to GP practices within Nottingham City, Nottingham North and East, Nottingham West and Rushcliffe Clinical Commissioning Group.

The service started on the 1st April 2019. It is a PICS operated service that provides a more joined up service for respiratory patients who are in hospital. The service works with hospital clinicians at both the City Hospital and QMC identifying patients who can leave hospital and ensuring the required ongoing care is provided in the local community health services. This provides a more integrated care for patients and aims to reduce avoidable re-admissions into hospital and supports a reduction in length of stay in hospital.
Greater Nottingham Commissioning Partnership believes that this service will help tackle the current pressure on respiratory services in local hospitals and also support future winter plans.
The pilot will run for approximately 20 months and within this period an evaluation will take place to assess how effective the service is at improving healthcare for respiratory patients. If the pilot proves successful, a full procurement exercise will be undertaken by Greater Nottingham Commissioning Partnership. PICS is funded to employ three respiratory nurses for this service.
