Provided for people registered to GP practices within Nottingham North and East and Nottingham West Clinical Commissioning Groups.
This is a community clinical pathway that integrates with local hospitals. The service has resulted in significant improvement in the management of patients with heart failure and fewer hospital admissions. The service is provided to meet the needs of patients diagnosed with left ventricular systolic dysfunction through echocardiogram (ECHO) which provides an ultrasound image of the heart.
A service for atrial fibrillation (AF) is also provided for Nottingham West Clinical Commissioning Group. atrial fibrillation is a relatively common condition that increases the likelihood of stroke. The increased detection of people with atrial fibrillation and their treatment using anti-coagulants significantly reduces the risk of stroke. This is an evidenced based approach supported by hospitals and public health to improve patient care in the community. The service is provided by a team of skilled nurses.
